Acknowledgements
1Standard Form of Agreement (SFOA)
The Services and/or Equipment are provided by COMSTEL ICT subject to its SFOA made available on the COMSTEL ICT website (comstel.com.au/legal). I have been given the opportunity to read, or I have read, and agree to abide by the terms and conditions set out in the SFOA.
2COMSTEL ICT — COMSTEL TV Box Terms & Conditions
By purchasing or using the Comstel COMSTEL TV Box, you agree to our Terms & Conditions, including no refunds unless the hardware is faulty, a 12-month included streaming licence, and the requirement to renew at $10 per month after expiry. Full Terms & Conditions here.
3Critical Information Summary (CIS)
Information about the Services and the fees and charges that apply are noted in the relevant CIS. I have received the applicable CIS and have been given the opportunity to read, or have read, the information contained therein. The CIS can be found online at comstel.com.au/legal. This application is subject to the conditions of the Telecommunications Consumer Protection Code.
4Early Termination
If the Service is acquired for a fixed term and the Service is cancelled before the expiry of the fixed term, an Early Termination Fee will be charged based on the remaining fixed term.
5Portability fees
In the event that any Voice or Mobile Services is transferred away to another service provider, I may be liable for Service number portability fees in accordance with the SFOA.
6About Comstel ICT
I understand and accept that COMSTEL ICT is not part of nor affiliated with any other telecommunications carrier, such as Telstra or Optus, and this was not implied at any stage.
7Privacy, Fair Use and Financial Hardship Policies
I have been made aware that the Services supplied are subject to the Comstel ICT Privacy Policy and Fair Use Policy, which are made available on the COMSTEL ICT website (www.comstel.com.au).
Important Notice to Purchasers
You are entitled to a 5 business day ‘cooling off period’ from the day you sign this agreement, during which time you have a right to cancel this agreement. Provisioning of services will not commence until after this cooling off period.
Declaration
I apply to COMSTEL ICT for the supply of the Services and/or Equipment indicated within this Application Form, subject to the COMSTEL ICT Standard Form of Agreement, and I authorise the transfer of any existing services and existing service numbers from my current provider at my own expense. I/we are a consumer as defined in the Telecommunications Protection (TCP) code. I/we acknowledge that if we are not a consumer as defined in the TCP Code, that code will not apply to the Application for Service.
